FREE Preventative Care in Your Neighborhood

The Michigan Department of Health and Human Services (MDHHS) has transitioned its former COVID-19 Testing sites into Neighbor Wellness Centers (NHWC’s) where two sets of services are offered to the community residents. Muskegon Family Care is pleased to partner with the NHWC program at the Christ Temple Church, located at 412 E. Sherman Blvd, Muskegon Heights, MI 49444 to provide FREE services focused on chronic disease prevention and detection. Services include: Diagnostic screening and testing for: COVID, hypertension, diabetes, and cholesterol.

The services are performed by a Muskegon Family Care Registered Nurse. The NHWC also provides Community Health Worker services that provide a matrix of services that intend to impact community residents’ upstream health status including: a brief health-related client assist, social determinants of heath (SDOH) intake screenings, health education and health coaching, health related social service referrals, and post assist or intake screening follow-up assistance. These programs ensure that all community residents’ SDOH needs are adequately addressed and met.

1) Do you have to be a patient of Muskegon Family Care to have services at the Neighborhood Wellness Center?

No, you do not have to be a patient of Muskegon Family to have testing, screening or CHW services.

2) Do you have to have medical insurance coverage?

No, services are provided free of charge.

3) Do you have to become a patient of Muskegon Family Care if screenings or testing determine additional medical care?

No, you do not have to establish Medical Care with Muskegon Family Care.

4) Is the Community Health Worker the same as a social worker?

No, the Community Health Worker is not a licensed social worker. The Community Health Worker serves as a bridge between individuals’ health-related knowledge and the health-related social resources that are available in their communities. Resources that can improve their healthy living knowledge and behaviors and address social determinants of health.
